Hot Weather Risk Assessment

As the summer progresses and the weather continues to surprise, we think it's important to take a person-centered approach when preparing for the possibility of a heatwave. This form helps to identify which persons are at increased risk during times of high temperature, the signs to look out for that signal heat related illnesses and what actions to take to decrease these risks.
